Provides face-to-face in-home and community support services to assist individuals in acquiring, maintaining, and improving self-help, domestic, socialization, and adaptive skills necessary to reside successfully in-home and community-based settings. This service will consist of support in the general areas of self-care, communication, fine and gross motor skills, mobility, personal adjustment, relationship development, socialization, and use of community resources.

IN-HOME AND COMMUNITY SUPPORT
COMPANION SERVICES
Provides companionship to individuals in a private home setting with the emphasis on supervision and assistance focused on the health and safety of adult individuals with intellectual disability.


CHORE SERVICES
Chore services consist of services needed to maintain the home in a clean, sanitary, and safe condition.

NURSING CARE
Diagnosing and treating human responses to actual or potential health problems through such services as case finding, health teaching, health counseling, provisions of care supportive to or restorative to life and well being, and executing medical regimens as prescribed by a licensed physician or dentist. The team does not include acts of medical diagnosis or prescription of medical, therapeutic, or corrective measures, except as may be authorized by rules and regulations jointly promulgated by the State Board of Medicine and the Board, which rules and regulations will be implemented by the Board.
HOUSE MAKER
Includes cleaning and laundry, meal preparation, and other general household care.

RESPITE CARE
Provides services for the respite in-home and out of home-related to providing supervision and support individuals in private homes. Our team will also provide enhanced and non-enhanced services for
Respite Care.
